What I do not understand is why they charge extra for the different sizes: $20 for a 45 mm pole and $40 extra for a 38mm.

Every pole is custom cut to fit your ceiling height and has a few inches of adjustment*.
I know it has been posted what that "several inches" is, but I cannot remember what it is. The thing is you do not want to order it too tall or it will have to be cut down.
