Pole Resume

I was poking around a website that is going to offer some MMA, Kickboxing classes etc in my area.  I read that they are thinking about offering some pole dancing classes.  So I sent them an email to see if to find out when they were going to start classes and if they had an instructor in mind.     I got a message back saying that they do not have an instructor yet but if I wanted to send my resume over they would love to see it.   What would I put on my pole resume? I do not have any real intstructor experience but lots of pole experience in general - esp competitive pole dancing.  Should I put down all the locations ive studied at, all my appearenaces, should I also link my You Tube site as well?    I havent applied for a job in over 7yrs (very lucky I know), so its almost starting over again.
